Individual Funding Request (IFR) Policy, Process and resources

BLMK CCG’s IFR Policy and Process sets out the principles of an IFR and how clinicians can apply for funding if their patient meets the criteria. A referring clinician can be from Primary, Community or Secondary Care. If a patient is in hospital, the treating clinician is able to refer for an IFR directly to BLMK CCG’s secure database system. The referring clinician is required to demonstrate on the IFR application form why their patient has clinically exceptional circumstances, the expected outcomes of the treatment and why they consider the treatment is a good use of NHS resources.
